Geriatrics

San Leandro Hospital, in collaboration with Alameda Health System, offers education, support, information, and assessment services for seniors, their families, and their caregivers. Work is done with older patients in ambulatory, long-term care and hospital-based settings.

Staff is trained to evaluate and manage the spectrum of geriatric syndromes, including dementia, delirium, falls/imbalance, depression, and developmental failures. They have the skills needed to provide individualized care in the basic domains of communication, symptom management, psychosocial support, and navigating through care settings within the community.

The team at San Leandro Hospital focuses on the function of older individuals, reviewing  the "geriatric vital signs" of mobility, mental status, continence, nutrition and integrity of the skin.

Senior care raises unique issues. Staff  is available to work with family members and caregivers with information and referrals on such topics as:
